About The Position

"Navitas Healthcare, LLC" is seeking a dependable Licensed Practical Nurse (LPN) for an evening shift assignment at a long-term care facility in Montour Falls, New York. The LPN will provide high-quality nursing care to residents while working closely with the healthcare team. The role requires recent nursing home experience and a willingness to assist with CNA duties as needed.

Requirements

• Active LPN license in New York
• Minimum 2 years of recent nursing home / long-term care experience
• Ability to work evening shifts consistently
• Willingness to assist with CNA responsibilities when required
• Strong patient care, communication, and teamwork skills
• Dependable and punctual
• BLS (Basic Life Support)

Responsibilities

• Deliver direct nursing care to residents in a long-term care setting
• Administer medications and treatments as prescribed
• Monitor patient conditions and communicate changes to supervising RN or physician
• Maintain accurate and timely patient documentation
• Assist with CNA duties as needed to support resident care
• Follow all facility protocols and infection control procedures
• Collaborate effectively with nurses, CNAs, and other healthcare staff
