About The Position

Kinston Community Health Center (KCHC) is seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to provide high-quality clinical support in an outpatient, team-based care environment. This role is ideal for an LPN who is passionate about patient-centered care, supporting providers, and contributing to positive health outcomes for a diverse community. The LPN plays a key role in advancing KCHC’s mission by assisting with patient care coordination, education, and clinical workflows that promote safe, efficient, and high-quality care. The Licensed Practical Nurse (LPN) at Kinston Community Health Center (KCHC) plays a key role in delivering high-quality, patient-centered care in a collaborative, team-based environment. The LPN provides direct clinical support by assisting providers with patient assessments, education, and follow-up care while ensuring compliance with all regulatory and organizational standards.

Requirements

  • Completion of an accredited Licensed Practical Nurse (LPN) program
  • Valid, unrestricted North Carolina Licensed Practical Nurse (LPN) license
  • 2-5 years of nursing experience in a healthcare setting, preferably in an FQHC or similar environment, preferred
  • Strong attention to detail and organizational skills.
  • Excellent verbal and written communication skills.
  • Ability to handle confidential information with integrity and discretion.
  • Clinical nursing skills within LPN scope of practice.
  • Understanding medical terminology and anatomy.

  • Bilingual in Spanish and English preferred

Responsibilities

  • Support and promote the mission, vision, and values of KCHC.
  • Maintain strict patient confidentiality and follow HIPAA/OSHA guidelines.
  • Participate in care team huddles and assist with new or complex patients.
  • Obtain medical histories, reconcile medications, and prepare patients for provider visits.
  • Provide patient education and follow-up care within nursing scope.
  • Collect, prepare, and handle specimens; sterilize instruments; dispose of supplies properly.
  • Assist with Medicare Annual Wellness Exams and complete screenings as directed.
  • Return patient calls, complete paperwork, and support provider efficiency.
  • Engage patients and families in care planning and connect them with community resources.
  • Respond to patient emergencies and maintain current BLS/CPR certification.
  • Participate in performance improvement, compliance programs, and ongoing professional development.
  • Perform additional duties as assigned.
