About The Position

Choices for People provides year-round support and instructional programs designed to aid individuals with disabilities in developing essential life skills. These skills promote inclusion, independence, self-sufficiency, and productivity; and aim to maximize the individual’s quality of life in the least restrictive environment possible. Choices for People assists in individual goal-oriented program planning, and acts as an advocate in the community through the removal of attitudinal, architectural, environmental, financial, employment-related, communicational, transportation, and other barriers. Position Summary The Licensed Practical Nurse (LPN) provides direct support and nursing-related services to individuals with intellectual and developmental disabilities. This position is responsible for supporting individuals on an assigned caseload by coordinating services, advocating for individual needs, and ensuring services are delivered in accordance with each individual’s Person-Centered Service Plan (PCSP), applicable regulations, and healthcare provider instructions. The LPN promotes independence, community inclusion, and person-centered outcomes while maintaining accurate documentation and ensuring compliance with organizational policies and state and federal requirements.

Requirements

  • Licensed Practical Nurse (LPN) license required (State of Missouri).
  • Minimum 1 year of nursing experience and experience working with individuals with IDD preferred
  • Ability to provide direct care and support services in a person-centered manner.
  • Knowledge of medication administration practices and ability to follow physician orders and established protocols.
  • Strong organizational skills with the ability to coordinate multiple services and priorities.
  • Ability to communicate effectively with individuals served, guardians, employers, community partners, and interdisciplinary team members.
  • Ability to maintain confidentiality and handle sensitive information in accordance with HIPAA and organizational standards.
  • Ability to complete accurate and timely documentation and maintain compliance with program and regulatory requirements.
  • Ability to work independently and collaboratively as part of a team.
  • Ability to safely assist individuals and perform direct support tasks as required by the work environment.
  • Ability to lift, carry, and move items or supplies as needed to support individuals served.
  • Ability to bend, reach, stand, and perform physical movements necessary to complete essential job duties.

Responsibilities

  • Establish and maintain a safe, secure, and positive environment that supports individuals’ needs, self-expression, and personal goals.
  • Provide individualized services, training, and support in accordance with healthcare professional instructions, the PCSP, and agency guidelines.
  • Support individuals with daily living needs, including routine errands and maintaining a clean, safe, and organized environment.
  • Promote independence in daily activities, including work, learning, leisure, and community participation.
  • Facilitate inclusion by supporting individuals in accessing and participating in community experiences and activities.
  • Coordinate and oversee services authorized by Vocational Rehabilitation (VR), Department of Health and Senior Services (DHSS), Veterans Affairs (VA), Department of Mental Health (DMH), or other funding sources as applicable.
  • Maintain knowledge of each individual’s requirements and service needs.
  • Communicate routinely with individuals, guardians, case managers, employers, and other stakeholders to address concerns, evaluate service delivery, and share program updates.
  • Advocate for individuals served to ensure needs are addressed and rights are supported.
  • Maintain accurate, current, and timely records in accordance with program requirements and organizational standards.
  • Stay current on regulatory requirements and best practices related to service delivery.
  • Participate in compliance reviews, audits, and other quality assurance activities as assigned.
  • Uphold all agency policies and standards and ensure compliance with applicable legislative and regulatory requirements.
  • Represent the organization professionally in the community and when interacting with stakeholders.
  • Operate agency vehicles and transport individuals as needed, in compliance with organizational policies and applicable state regulations.
  • Administer medications as prescribed and in accordance with agency policy, physician orders, and applicable regulations.
  • Provide medical care and health-related support as outlined in each individual’s PCSP.
