About The Position

"Navitas Healthcare, LLC" is seeking a dedicated Licensed Practical Nurse (LPN) to provide quality nursing care in a long-term care setting . The LPN will support resident health and wellbeing by administering medications, performing treatments, documenting care, and coordinating with physicians and care teams while supervising nursing assistants during assigned shifts.

Requirements

  • Graduate of an accredited Licensed Practical Nurse (LPN) program
  • Active and unrestricted Massachusetts LPN License
  • BLS / CPR / AED Certification
  • Experience in Long-Term Care or Skilled Nursing preferred
  • Experience with medication administration and patient charting
  • Strong clinical and patient care skills
  • Ability to supervise and support nursing assistants
  • Excellent documentation and electronic charting skills
  • Strong communication with residents, families, and healthcare teams
  • Ability to work independently and make sound clinical decisions
  • Ability to lift up to 50 lbs and meet physical job requirements

Responsibilities

  • Provide direct nursing care according to physician orders and care plans
  • Administer medications and monitor residents for reactions or changes
  • Monitor and document vital signs (temperature, pulse, respiration, blood pressure)
  • Perform treatments including wound care, catheter care, tube feeding, suctioning, and dressing changes
  • Assist residents with therapeutic and mobility-related care
  • Supervise and guide nursing assistants during assigned shifts
  • Coordinate daily care activities with nursing and support staff
  • Ensure adherence to facility policies and nursing standards
  • Maintain accurate patient charts and nursing documentation
  • Record resident conditions, treatments, and responses to care
  • Update medication administration records and treatment plans
  • Document admissions, transfers, and discharges as required
  • Communicate with physicians regarding patient status and treatment plans
  • Receive and document physician orders
  • Participate in care discussions and provide updates during rounds
  • Collect laboratory specimens such as blood, urine, and sputum
  • Assist with routine laboratory testing and documentation of results
  • Educate residents and families regarding care plans and health needs
  • Support resident orientation and adjustment to the facility environment
  • Maintain confidentiality of resident health information (HIPAA compliance)
  • Support regulatory surveys and facility inspections when required
  • Ensure adequate availability of medications and nursing supplies
