About The Position

The Behavioral Health Licensed Practical Nurse (LPN) is a part-time position (24 hours per week) that supports the delivery of outpatient behavioral health services through an integrated care model. This role works collaboratively with behavioral health providers, medical professionals, and community partners to support individuals receiving mental health services. The LPN provides clinical support, coordination of care, patient education, and consultation to ensure high-quality, person-centered services. The position requires clinical judgment, strong collaboration skills, and the ability to function effectively within a multidisciplinary behavioral health environment.

Requirements

  • Demonstrated clinical nursing experience in behavioral health, psychiatric, or related healthcare settings.
  • Strong working knowledge of psychiatric disorders, psychopharmacology, and integrated care models.
  • Ability to collaborate effectively within multidisciplinary teams and with community partners.
  • Strong organizational, documentation, and time management skills.
  • Effective verbal and written communication skills.
  • Ability to work independently while maintaining accountability to program leadership.
  • Must meet DCFS/CANTS Clearance Requirements.
  • Must be at least 21 years of age.
  • Must possess and maintain a valid driver’s license and maintain proof of automobile insurance.
  • Current, active Illinois Licensed Practical Nurse (LPN) License required.
  • Completion of a state-approved practical nursing program required.
  • Completion of CARE and TCI training is an internal requirement for all staff and must be completed upon hire.
  • Pass the written test annually
  • Pass the physical test every six months (if applicable to their role)

Nice To Haves

  • Minimum of two years of clinical nursing experience preferred, with experience in psychiatric or behavioral health setting strongly preferred.

Responsibilities

  • Provide nursing support to outpatient behavioral health clients, including assessment of mental and physical health needs.
  • Conduct assessments, monitor symptoms, and evaluate overall client well-being.
  • Collaborate with the integrated healthcare team to support the development and implementation of individualized treatment plans.
  • Provide medical consultation and guidance to team members as appropriate for client care.
  • Administer and monitor psychiatric medications as prescribed, including tracking effectiveness and potential side effects.
  • Provide psychoeducation and skill-building interventions to support client stability and wellness.
  • Coordinate care with psychiatrists, primary care providers, therapists, and other internal and external partners.
  • Educate clients and, when appropriate, families on mental health conditions, treatment options, medications, and available community resources.
  • Assess and respond to crisis situations, including coordination of referrals to higher levels of care when needed.
  • Promote health education, preventative care, and wellness screenings related to mental and physical health.
  • Maintain accurate, timely, and compliant documentation within the electronic health record.
  • Advocate for clients to ensure access to appropriate services and support.
  • Participate in ongoing professional development and continuing education related to psychiatric nursing and behavioral health.
  • Practices in compliance with the Illinois Nurse Practice Act and all applicable licensure requirements.
  • Perform other duties as assigned by the Director of Behavioral Health.
