As the Licensed Social Worker (LSW), you are the bridge between patients, families, and the resources they need. You’ll assess psychosocial needs, provide counseling, and collaborate with an interdisciplinary team to ensure every patient receives holistic, dignified care. Requirements

  • Bachelor’s degree in Social Work from a CSWE-accredited program
  • Minimum 1 year in healthcare; hospice experience strongly preferred
  • Deep understanding of hospice philosophy and end-of-life care
  • Current CPR certification required
  • Valid driver’s license, auto insurance, and reliable vehicle for community visits

Responsibilities

  • Comprehensive Assessments: Evaluate psychosocial status of patients and families related to terminal illness and living environment
  • Care Planning: Develop and implement interventions based on thorough evaluations
  • Counseling & Support: Guide patients and caregivers through stress, coping challenges, and end of life issues
  • Documentation: Maintain accurate, timely clinical records for all referred patients
  • Resource Navigation: Provide referrals and serve as a liaison with community agencies
  • Team Collaboration: Participate in interdisciplinary care planning and regular team meetings
  • Advocacy: Help physicians and team members understand social and emotional factors impacting care
  • Quality Improvement: Actively contribute to performance improvement initiatives
  • Flexibility: Perform additional duties as delegated by the Clinical Supervisor
