Job Description: Licensed Practical Nurse (LPN) Home Health | Valparaiso, IN At Elara Caring, we care where patients live. Requirements

• Active LPN license in the state of Indiana
• At least one year of clinical nursing experience
• Reliable transportation, valid driver’s license, and insurance
• Ability to lift, push, pull, or support up to 50 pounds

Nice To Haves

• Home health or hospice experience preferred

Responsibilities

• Provide skilled nursing care under the direction of an RN or physician
• Follow and verify the patient plan of care during each visit
• Document skilled nursing visits and patient progress
• Communicate changes in patient condition to the care team
• Educate patients and caregivers on self care and safety
