The Life Plan Coordinator for Meadowood at Home will successfully manage the needs of members and deliver appropriate services. Responsibilities include working in conjunction with the program manager to develop a network of sub-contracted service providers. Interviews, selects, and negotiates contracts with all service providers including home care providers and transportation services. Must obtain and review documents for respective service providers to ensure they meet Meadowood at Home standards. The position requires travel to visit members in hospitals, rehab centers, or at home to determine need for services and to provide reassurance.
