About The Position

Schedule: Part-time/Day Shift 3 days per week Tuesday/Thursday and 3 rd day still be to determine as we add an additional location will determine the added day No Weekends/Holidays Requires travel- 2x per month to Paoli, 2x per month to Greene County as a minimum Work duties will include case management for care-related needs such as connecting clients to OB services when needed, Primary Care when needed, and specialty services such as Hep C/HIV when appropriate. Responsibilities also include performing urine drug screens, conducting vital assessments (e.g., blood pressure and weight), and providing follow-up related to medication and any additional health needs identified. Additionally, there may be opportunities to attend and support clients during medical appointments upon request. Description : Knows and uses the nursing process in planning, implementing, and evaluating nursing care to individual patients. Collaborates with other members of the healthcare team. Evaluates response to interventions and patient status toward goal achievement. Accepts delegated nursing measures for which he or she knows he or she is prepared, qualified and licensed to perform.

Requirements

  • Requires graduation from a nationally accredited licensed practical nursing program
  • Requires an active Licensed Practical Nurse (LPN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) LPN license
  • Requires Basic Life Support certification through the AHA
  • Successful completion of orientation required

  • 1-2 years of relevant experience preferred
