Requirements

  • Graduate of LPN Program
  • Strong Customer Service Skills
  • Excellent written and verbal communication skills
  • Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms
  • Licensed Practical Nurse
  • BLS/CPR certification by American Heart Association is required within 60 days of hire

Responsibilities

  • The LPN, Face to Face Liaison obtain, track and monitor face to face documentation to ensure compliance with Medicare regulations.
  • Act as liaison between Home Care services and Intake Department.
  • Review clinical documentation from physician offices, hospitals and facilities as it relates to face to face encounters.
  • Collect all face to face documentation for all Medicare referrals.
  • Responsible for ongoing compilation from Soarian, Sigma Care, physician office and any other systems deemed necessary.
  • Document in appropriate system.
  • Monitor and obtain face to face documentation from physicians according to regulatory guidelines for patients admitted to home care services.
  • Communicate with all Intake staff members to ensure all face to face documentation is collected timely, accurately and documented according to the established process.
  • Responsible for running daily face to face system reports and any other reports as needed to ensure accurate collection of outstanding face to face documentation.
  • Update and maintain face to face log.
  • Collaborate with operations face to face liaisons through daily open communication in order to facilitate an increased completion rate.
  • Other duties and/or projects as assigned.
  • Demonstrate flexibility with job responsibilities at all times.
  • Adheres to HMH's Organizational competencies and standards of behavior.
  • Lifts a minimum of 10 lbs., pushes and pulls a minimum of 10 lbs. and stands a minimum of 4 hours a day.
