As an LPN on our Weekends team, you will: • Deliver 1:1 skilled nursing care • Work collaboratively with RN Case Managers • Practice at the top of your license • Provide complex clinical treatments • Support patients recovering from surgery, illness, or chronic disease exacerbations You won’t “manage” patients — you’ll focus on delivering high-quality, hands-on clinical care and education that prevents hospital readmissions and improves lives. Primary coverage will be in: • South of Boston to include lower Middlesex, Suffolk and Norfolk counties While navigating transportation and parking in the city can be challenging, we will secure city parking passes for new team members to make the commute as convenient and stress-free as possible! Schedule & Workload • Part-time (20 hours) preferred • Saturday-Sunday schedule, 8:30am - 6:30pm • Average of 8 visits per day • Dedicated time for documentation, care coordination, and chart review No hospital shift chaos. No overnight rotations. Just meaningful, focused patient care! Job Summary Provide quality nursing care to patients in various settings in the community. Patient care is delivered within MGB Home Care's philosophy, policy, and standards of community health practice. In collaboration with the RN case manager this position will provide skilled nursing visits, utilizing thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures and standards of practice.
