About The Position

JOB SUMMARY: A qualified Licensed Practical/Vocational Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. Services are furnished in accordance with Agency policies. QUALIFICATIONS: 1. Graduate of a state approved school of practical (vocational) nursing and currently licensed in the state of Florida. 2. Minimum of one (1) year experience in nursing, preferred. 3. Acceptance of philosophy and goals of this Agency. 4. Ability to exercise initiative and independent judgment. RESPONSIBILITIES: Florida Specific: 1. A licensed practical nurse shall be licensed in the state of Florida and provide nursing care under the direction of a registered nurse who provides on-site supervision as needed based upon the severity of the patient's medical condition and the nurse’s training and experience. 2. The licensed practical nurse will be supervised by the registered nurse, and the visits will be documented in the patient record. 3. The licensed practical nurse will have an annual evaluation by a registered nurse. 4. The Licensed Practical Nurse or Licensed Vocational Nurse (LPN/LVN) will supplement the nursing care needs of the patient as provided by the Registered Nurse and include: Providing services in accordance with Agency policies. Preparing clinical and progress notes. Assisting the Registered Nurse or physician in performing specialized procedures and duties. Assisting the Registered Nurse in carrying out the plan of care. Assisting the patient in learning appropriate self-care techniques. 5. The LPN/LVN will be supervised by the RN as required by applicable state rules and regulations. In Florida, the registered nurse will perform an annual evaluation on the LPN 6. Understands and adheres to established policies and procedures. 7. Implements the nursing care plan for each patient. 8. Provides nursing services, treatments and diagnostic and preventive procedures as assigned. Initiate preventive and rehabilitative nursing procedures as appropriate. 9. Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient’s physical or emotional condition. a. Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home. 10. Evaluates with registered nurse the effectiveness of the LPN’s/LVN’s nursing service to the patient and family under the guidance of the registered nurse. 11. Maintains accurate and complete records of observations, treatments and care of patients. 12. Participates in medical record audit as assigned. 13. Attend staff meetings, patient care conferences and in-services as scheduled. 14. Takes on-call duty, nights, weekends and holidays assigned. 15. Is responsible for: Submitting any changes in schedule to Director of Nursing/Clinical Manager daily. Participating in patient care conferences to discuss the need for involvement of other members of the health team, such as physical therapist or speech language pathologist. 16. Prepares clinical and progress notes. 17. Assists the physician and RN in performing specialized procedures. 18. Prepare equipment and materials for treatments. 19. Observes aseptic technique as required. 20. Assists the patient in learning appropriate self-care techniques. WORKING ENVIRONMENT: Works indoors in Agency office and patient homes and travels to/from patient homes. JOB RELATIONSHIPS: 1. Supervised by: Director of Nursing/ Clinical Manager/RNs RISK EXPOSURE: High risk LIFTING REQUIREMENTS: Ability to perform the following tasks if necessary: Ability to participate in physical activity. Ability to work for extended period of time while standing and being involved in physical activity. Heavy lifting. Ability to do extensive bending, lifting and standing on a regular basis.
