About The Position

This position is office-based and supports key home health operations, including referral and intake processes, to help ensure timely and coordinated patient care. Must be self-directed and able to perform with a high level of accountability and minimal supervision. Functions as a resource for employees and clients. Performs clerical and managerial duties in support of multiple departments and leaders.

Requirements

  • LPN required.
  • Must have at least three years of applicable experience, with previous experience in a healthcare setting preferred.

Responsibilities

  • Establishes and maintains good interpersonal relationships.
  • Respects and maintains confidentiality of all verbal and written communications.
  • Considerable initiative and judgment is involved in assuming responsibility for routine matters, answering inquiries.
  • Responsible for specific tasks requiring data entry, department ordering, time and attendance, production improvement and phone services.
  • Provides project support as needed.
  • Provides training and orientation to new staff.
  • Knowledge of the principles and skills needed for nursing to provide patient/resident care and treatment.
  • Knowledge of medications and their effects on patients/residents.
  • Knowledge of patient/resident education principles to develop a self-care program.
  • Knowledge of common safety hazards and precautions to establish a safe environment, specific to the home setting.
