Requirements

HS Diploma or Equivalent
Current LPN License, Liability Insurance
Min. 1 year experience in a licensed health care facility.
Push, pull,squat, turn, pivot, transfer and lift at least 50 pounds.
Sit, Stand and/or Walk for extended period of time while providing physical assistance to consumer

Nice To Haves

Prior experience working with the elderly and/or adults with disabilities preferred.

Responsibilities

Administer medication to consumer according to individual plan of care, physician's orders and emergent needs, and document clinical and progress notes on members' medical record.
Perform assessments and observing and monitoring consumer's vital signs and responses to treatments and medications, reporting abnormalities or concerns.
Coordinate care with other consumer care services (ie: Dietary, Pharmacy, Dental, Rehabilitation, Physical/Speech/Occupational Therapy, Audiology, DME).
Assist consumer with daily functional activities, including but not limited to, hygiene, meals, ambulation and social activities.
