About The Position

Professional supervision and direction is provided by the facility physician and/or Registered Nurse as assigned. Daily scheduling and direction is provided by the Facility Director. The LPN provides support to the physician in scheduling, assessments, obtaining orders and medication, maintaining documentation. The LPN passes medication and maintains accurate records of all professional activities. The LPN serves as resource for staff and residents and works a regular schedule and “on call”.

Requirements

  • Excellent attention to detail in completing work tasks
  • Ability to be a team player and work with diverse populations.
  • Ability to lead, train, coach, and develop others
  • Excellent instructional skills
  • Ability to create curricula based on program needs
  • Requires graduation of an accredited Licensed Practical Nurse program
  • Valid West Virginia LPN License
  • Must have Pharmacology certificated.
  • Must have valid driver’s license
  • Able to pass drug testing, fitness for duty, and background check

Nice To Haves

  • Prefers 1-2 years’ LPN experience.

Responsibilities

  • Physician Authorizations Responsible for obtaining physician signatures as required on all documents.
  • Pass medications as ordered by the physician.
  • Prepare MAR cards for use
  • Review and certify MAR cards are completed properly.
  • Keep track of all med orders and any new orders on the MAR cards.
  • Update MAR cards on all new medication orders and remove old orders from the card.
  • Audit MAR cards for compliance with orders.
  • Check for any missed medications.
  • Follow up with any missed meds with the appropriate staff.
  • Organize, schedule and document client’s medical assessments,
  • As identified by the Physician obtain medication, schedule procedures, lab work etc. and follow up to insure client’s needs are met.
  • Review medications with residents and ensure they are aware of possible side effects.
  • Collect health care reports, physicians’ orders, document future appointments and enter the information and track it on the client’s discharge summary.
  • Make note of any medication changes or treatments in the Communications Log.
  • Monitor, document and follow-up on specialist or emergency room visits.
  • Maintain a regular “on call” schedule.
  • Work a flexible schedule as determined by the needs of the clients and availability of the physician.
  • Employees are required to use their personal vehicles in certain circumstances and as directed in the performance of their job duties.
