About The Position

We're looking for a compassionate and dedicated Home Health LPN to become part of our expanding team. In this role, you'll provide skilled nursing care to patients in their homes on an intermittent basis. Mileage Reimbursement Company Paid Tablet Flexible Schedule We pay for CPR Recertification Brief overview of Responsibilities: Support the Registered Nurse in delivering each patient’s plan of care Assist with specialized procedures under the RN’s direction and the physician’s orders Contribute to planning, implementing, and regularly evaluating patient care plans Recognize how social and economic challenges impact patients, and provide emotional and physical support for them and their families Help educate patients and families on proper self-care techniques Actively participate in team and staff meetings to promote coordinated care Document clinical and progress notes after each patient visit Monitor and report any symptoms, reactions, or changes in patient condition to the appropriate team member Participate in a rotating weekend on-call schedule Perform other duties as needed

Requirements

  • Must be a Licensed Practical Nurse with a current license to practice nursing in the Commonwealth of Virginia
  • Graduate of an accredited nursing school.
  • Must have a valid driver's license and a reliable car available for use during work hours.

Nice To Haves

  • At least one year of home health experience preferred
