Requirements

  • Licensure: Current, valid Licensed Vocational Nurse (LVN) License in California.
  • Experience: 1–3 years of supervisory or case management experience in a long-term care setting is preferred.
  • Technical Skills: Proficiency in Microsoft Office Suite; experience with PointClickCare (PCC) is highly preferred.
  • Soft Skills: Must be an independent decision-maker with the ability to deal tactfully with personnel, residents, families, and government agencies.
  • Education: Graduation from an accredited School of Nursing (BSN preferred).

Responsibilities

  • Clinical Management: Assist the DNS in planning, organizing, and evaluating daily nursing functions. Perform daily rounds to ensure nursing standards are consistently met.
  • Care Planning & MDS: Partner with the Resident Assessment Coordinator to develop, schedule, and revise comprehensive care plans (MDS/RAPs). Ensure plans are descriptive, goal-oriented, and involve input from residents and families.
  • Quality Assurance: Participate in the facility’s QA program, identifying areas for improvement and implementing corrective plans of action.
  • Patient Advocacy & Resolution: Act as a liaison to resolve issues or concerns raised by residents, patients, or family members regarding care.
  • Administrative Oversight: Maintain current reference materials (PDRs, standards of practice) and ensure the Nursing Service Procedures Manual is up-to-date.
  • Staff Education: Assist the In-Service Director in developing annual training programs, including OSHA, HIPAA, and Infection Control.
  • Staffing Support: Perform general nursing duties during emergencies or staffing shortages to ensure resident safety.
