About The Position

Primary Duties and Responsibilities Under the direction of a registered nurse or physician and under assigned administrative supervision, provides care within the scope of the Vocational Nurse Practice Act and regulatory standards. Assists in the assessment, planning, implementation and evaluation of patient care. Follows patient identification procedures, provides appropriate information for all patient procedures, and collects and handles specimens. Assesses and implements prescribed interventions for the patient. Implements and documents discharge plan for each patient. Recognizes that ethical/legal/medical dilemmas exist in the nursing practice and confers with the appropriate resource personnel when making ethical nursing decisions. Participates in maintaining a safe patient care environment. Assist with the care and maintenance of department equipment. Complete and accurate charting of patient records. Complete and accurate record keeping of equipment and quality measures. Collaborate with other disciplines of the health care team to ensure the delivery of care across the continuum, providing timely, safe and quality care. Responsible for providing a safe, HIPAA compliant environment for the patient and the staff. Performs other duties as assigned. Primary Care and Urgent Care: Assist medical personnel with special procedures, minor surgical procedures and/or diagnostic exams. The LVN will process in office tests, collect specimens, and perform procedures within the scope of the Vocational Nurse Practice Act and regulatory standards. Qualifications Education and Experience: High School diploma or equivalent. One or more years of experience in a healthcare care facility (Specialty Clinics can hire a new grad LVN with no experience) Basic keyboarding skills and knowledge of Microsoft Office Suite, including Outlook, Word and Excel preferred Basic knowledge of medical terminology preferred Experience with Electronic Medical Records preferred Home Health: At least one (1) year of experience in the area of public health or home care nursing. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Hospice: Community health/hospice or medical/surgical experience is preferred. Understands philosophy of hospice concept. Also understands needs of the terminally ill. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Primary Care and Urgent Care: Completion of educational courses (within 6 months of hire): New Hire LVN course – Primary Care or Urgent Care

Requirements

  • High School diploma or equivalent.
  • Current LVN License to practice in the State of California
  • Current BLS Certification
  • Completion of educational courses (within 6 months of hire): New Hire LVN course – Primary Care or Urgent Care
  • At least one (1) year of experience in the area of public health or home care nursing (Home Health)
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order (Home Health and Hospice)

Nice To Haves

  • One or more years of experience in a healthcare care facility (Specialty Clinics can hire a new grad LVN with no experience)
  • Basic keyboarding skills and knowledge of Microsoft Office Suite, including Outlook, Word and Excel preferred
