Reporting to the Director, Post-Acute & Advanced Care, the Manager, Advanced Care is responsible for leading and operationalizing advanced care initiatives, with a primary focus on Shared Decision-Making (SDM), care goals planning, and end-of-life alignment across the organization. This role provides strategic and operational oversight for large, complex, cross-functional initiatives throughout the full project lifecycle. The Manager, Advanced Care will drive the overall direction, coordination, implementation, execution, and ongoing optimization of multiple concurrent initiatives, ensuring alignment with Value-Based Care strategies, clinical best practices, regulatory standards, and organizational goals. This position plays a critical role in partnering with physicians, specialty teams, care management, and external partners to normalize advanced care conversations, improve patient experience, and support thoughtful utilization management.

Requirements

  • Bachelor’s degree in management, business, or related field required
  • 3+ years’ previous experience in project management working with multiple departments and stakeholders (healthcare projects preferred)
  • Work efficiently, self-motivated, fast learner and strong work ethic.
  • Strong written and verbal communication skills, including the ability to communicate complex issues to varying audiences and levels
  • Ability to manage in a matrix environment
  • Familiar with project management methodology and techniques and change management
  • Experience in PM methodologies
  • Experience working with vendors
  • Advanced knowledge of Microsoft Office Products (MS Word, MS Excel, MS Visio, MS Power Point, MS One Note, MS Project and D365 )
  • Experience with compiling and following strict budgets
  • Excellent verbal and written communication skills
  • Able to multi-task, prioritize, and manage time effectively

Responsibilities

  • Lead, develop, and manage complex, cross-functional initiatives that support the Shared Decision-Making (SDM) program, advanced care planning, and broader Post-Acute & Advanced Care priorities.
  • Drive the successful delivery of multiple concurrent initiatives, ensuring alignment with Value-Based Care strategy, clinical best practices, and organizational goals.
  • Establish project governance, controls, standards, timelines, and success criteria across initiatives.
  • Own the day-to-day execution of assigned projects, ensuring progress from planning through implementation and optimization.
  • Support the design and rollout of new workflows, pilots, and operational models that improve patient experience, provider support, and care alignment across the continuum.
  • Communicate clear objectives, expectations, and progress with physicians, clinical leaders, executives, sponsors, and cross-functional partners.
  • Serve as a key liaison between internal teams (Care Management, Specialty, Operations, Analytics) and external partners (e.g., hospice, home health, clinical liaisons).
  • Develop and maintain comprehensive project documentation, including charters, scopes, milestones, deliverables, and implementation plans.
  • Provide regular project status reporting, including timelines, risks, dependencies, and mitigation strategies.
  • Support physician engagement and adoption by partnering on communication strategies, education plans, and feedback loops.
  • Identify, manage, and resolve project risks, issues, and interdependencies to ensure timely and effective execution.
  • Ensure initiatives are delivered on time, within scope, and aligned with defined objectives.
  • Develop and execute mitigation plans when initiatives are off track.
  • Support and enhance project methodologies and tools to improve efficiency, scalability, and resource utilization.
  • Partner with analytics and operational leaders to define, track, and refine performance metrics tied to SDM, advanced care planning, post-acute outcomes, and value-based performance.
