Work Shift: 8 Hours - Day Shifts (United States of America) Scheduled Weekly Hours: 40 Union Position: No Department Details The Manager of Case Management will lead day to day operations of the inpatient case management team, work collaboratively with other care management leaders in the inpatient, post acute, and ambulatory locations, and with nursing, providers, and interdisciplinary teams to support staff and safe transitions for patients. The manager will have a focus on efficient movement of patients through the inpatient stay, eliminating barriers to discharge, and team collaboration to support capacity and throughput at the medical center. The inpatient case management department is part of the Sanford Nursing structure, represented at nursing senate, committees, and interdisciplinary teams. Summary Provide leadership and day-to-day management of the case management efforts within Sanford. Leads case management initiatives in different settings and environments. Enhance the throughput of patients and reimbursement received through appropriate status utilization, coordination of care, length of stay management, and discharge disposition management to ensure the delivery of quality healthcare and development of services according to the mission, vision, and strategic goals of Sanford.
