About The Position

Heartland Alliance Health’s (HAH) mission is to transform healthcare for the most vulnerable – particularly people experiencing homelessness, mental illness or addictions, or struggling with multiple chronic illnesses – improving health for all and the well-being of our community. The Manager of Grants Development is responsible for contributing to all aspects of the organization's resource development strategy, including identifying, cultivating, soliciting, and stewarding funding from federal, state, and local government sources, private foundations, corporate partners, and individual donors. This position plays a critical role in ensuring the financial sustainability and growth of community health programs.

Requirements

  • Bachelor's degree in social work or related field with a Master’s degree preferred
  • Experience with writing or managing grants is required.
  • Experience working in a health care, HRSA, CDPH, or managing public grants.
  • Ability to write planning documents, reports, grant applications and business correspondence.
  • Proficient in Microsoft Excel and mathematical concepts such as probability, percentages and ratios.
  • Ability to effectively communicate with diverse stakeholders and staff.
  • Ability to meet deadlines, develop realistic action plans and measurable outcomes, create program budgets, prioritize and plan workload, and use time efficiently.
  • Ability to interpret scientific, technical and financial reports and government and legal documents.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.

Responsibilities

  • Identify and research funding opportunities from government agencies (HRSA, SAMHSA, CDC, state and local sources), private foundations, corporations, and other philanthropic entities
  • Manage a portfolio of grants throughout the grant lifecycle, including preparing applications, proposals, and reports across multiple funding streams
  • Create and maintain a grants calendar tracking deadlines, reporting requirements, and renewal dates
  • Ensure compliance with all grant requirements, regulations, and reporting standards
  • Monitor grant performance metrics and outcomes to demonstrate impact to funders
  • Write compelling grant proposals, letters of inquiry, concept papers, and funding requests
  • Collaborate with clinical, financial, operational, and program staff to develop evidence-based program designs and budgets
  • Articulate organizational needs, community health data, and program outcomes in clear, persuasive narratives
  • Prepare grant budgets in coordination with finance department, ensuring accurate cost projections and allowable expenses
  • Customize proposals to align with specific funder priorities, guidelines, and interests
  • Research and cultivate relationships with foundation program officers and corporate giving representatives
  • Prepare briefing materials and talking points for executive leadership meetings with funders
  • Represent the organization at funder meetings, site visits, and community forums
  • Develop tailored solicitation strategies for institutional funders based on giving capacity and interests
  • Design and implement an individual giving program, including annual fund campaigns, major gift solicitation, and planned giving initiatives
  • Create donor communications, including appeal letters, acknowledgment correspondence, and impact reports
  • Identify, qualify, and cultivate individual donor prospects through research and relationship building
  • Partner with board members and leadership to engage donors and secure financial support
  • Maintain accurate donor records and ensure timely gift acknowledgment and stewardship
  • Prepare comprehensive grant reports documenting program activities, outcomes, and financial expenditures
  • Develop impact stories, case studies, and data visualizations to demonstrate program effectiveness
  • Maintain strong relationships with existing funders through regular communication and transparency
  • Coordinate funder site visits and presentations showcasing organizational programs and impact
  • Ensure proper donor recognition through annual reports, website acknowledgment, and special events
  • Maintain grants management database and donor CRM system with accurate, up-to-date information
  • Track fundraising metrics including proposal submission rates, success rates, and revenue projections
  • Generate reports on fundraising activities and outcomes for leadership and board review
  • Ensure compliance with data privacy regulations and ethical fundraising practices
  • Work closely with executive leadership, finance, clinical, and program teams to align fundraising with organizational priorities
  • Provide guidance and support to program staff on grant-funded activities and requirements
  • Serve as staff liaison to board development committee
  • Supervise development staff and volunteers as the team grows
  • Participate in strategic planning processes and organizational leadership meetings
