About The Position

Are you a CDCA-certified professional in search of new opportunity? The Crossroads Center is seeking a compassionate and detail-oriented MAP Care Coordinator to provide essential case management services to women in Ohio facing substance abuse challenges – many of whom are pregnant or living with persistent mental health conditions. As a MAP Care Coordinator, you’ll work closely with clients and treatment providers to support recovery and ensure successful outcomes through our collaborative, client-centered approach. Requirements

  • Chemical Dependency Counselor Assistant (CDCA) Certification in the state of Ohio is required.
  • Valid Ohio driver’s license and insurance.
  • Ability to pass a background check and drug screen.
  • Tuberculosis Skin Test and Influenza (Flu) vaccination within the past year;
  • Ability to work a shift of 5:30 a.m. – 2:00 p.m. (M-F).
  • Ability to frequently lift or move objects weighing up to 15 lbs.

Nice To Haves

  • Bachelor’s Degree in psychology, social work, or related field is preferred.

Responsibilities

  • Provide clinical therapeutic and case management services for adults with psychiatric disabilities to assist them in improving their current level of functioning in the community.
  • Complete comprehensive assessments for diagnosis and evaluation of level of functioning, support network, adequacy of living arrangements, financial status, physical health, and level of self-care.
  • Assist clients in identifying needs, setting goals, establishing concrete objectives, and developing a coordinated care plan with a set timeframe. Ensure their access to local resources and assist members in applying for benefits and health insurance such as SSI, SSDI, Medicaid, or Medicare.
  • Actively reach out to clients who have not been in contact to ensure engagement in services in their home or transitional housing placement.
  • Outreach and engage potential clients to evaluate appropriateness and desire for services in jails, hospitals, or street settings.
  • Provide needed therapeutic interventions and conduct support groups to help clients improve their level of functioning.
