HOPE ~ HEALTH ~ HEALING Terros Health is hiring a MAT Case Manager at our Stapley Dr Heath Center in Mesa, AZ. Location: 1111 S Stapley Dr #5060, Mesa, AZ 85204 Full-Time, Employed Mon - Fri, 8:30am - 5:00pm Flexible Scheduling Full Benefits, Including 401K, and Generous PTO/PST (4+ weeks/yr.) Additional Language Differential Pay Available! Under the Agency’s policies and professional requirements, participates as a member of an interdisciplinary clinical team working with individuals diagnosed with an opiate use diagnosis enrolled in medication assistance treatment. Manages those identified clients who receive MAT services. Completes assessments, crisis plans, and individualized service plans. Communicates and documents client progress toward their recovery. Embraces the recovery philosophy and promotes a safe recovery environment for clients. Uses brief clinical interventions, motivational Interviewing, and supportive listening skills to engage and support clients receiving services. Works under the direct supervision of the Regional Administrator or Clinical Manager.

Requirements

  • Meet BHT requirements and has a high school education or GED
  • A bachelor’s degree in the field of behavioral health and any behavioral health experience, or A master’s degree or higher in the field of behavioral health preferred.
  • Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members.
  • Excellent oral and written communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions.
  • Basic understanding of the DSM-5 preferred and experience working with opiate use disorder/substance abuse patients.
  • Proficient in Microsoft Word, Excel, PowerPoint, Outlook.
  • Experience in electronic medical record, NextGen preferred.
  • Ability to work independently, highly organized, and capable of managing multiple ongoing projects.
  • Must have valid Arizona driver’s license, be 21 years of age with minimum 3 years driving experience, and meet requirements of Terros Health’s driving policy.
  • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
  • Must pass a TB Test.

Responsibilities

  • Works collaboratively with the clinical team to engage, educate, communicate, and coordinate care with clients receiving medication assistance services for MAT program.
  • Conducts outreach and follow-up of services including, but not limited to, crisis interaction, missed appointments, and hospital discharges to ensure adequate resources are available and in place.
  • Ensures ongoing collaboration, including the communication of appropriate clinical information with other involved parties as appropriate.
  • Coordinates care with a client’s family, behavioral and general medical providers and other involved supports including educational, social, judicial, and other State agencies.
  • Participates in clinical staffing’s, case conferences, or other meetings, with or without the client or his/her family or other natural support participation.
  • Provides excellent customer service to clients, internal and external provider agencies, State entities, or other stakeholders (e.g. probation officers, advocates, family members, hospital staff, jail staff, vocational rehabilitation, etc.) as needed.
  • Performs all case management functions associated with caseload including completing the assessment and service planning processes; including identifying the need for further or specialty services.
  • Provides continuous evaluation of the effectiveness of treatment through the ongoing assessment of the client, input from the person and relevant others, and consultation with the clinical team, resulting in modification to the service plan as necessary.
  • Ensures documentation in the Electronic Health Record is completed the same day Service Delivery is provided.
  • Consistently meets Service Delivery expectations, which are no less than 62.5% of paid time.
