A growing company with opportunities for advancement Company sponsored training, tuition reimbursement, and other learning opportunities Job Description The MDS Coordinator is responsible for the overall coordination of the facility Medicare, Resident Assessment Instrument (MDS) and care planning process.

Requirements

  • Graduate of accredited nursing program
  • Must have a valid, unrestricted RN license in the state

Nice To Haves

  • Previous MDS experience preferred
  • Previous experience in Long Term preferred, but not required

Responsibilities

  • Assess and evaluate the quality of care being given to long-term care residents
  • Perform the initial interview and assess new residents
  • Develop a care plan that has long-term and short-term goals for improvement
  • Conduct meetings with multidisciplinary treatment teams, caretakers, and families
  • Reach out to specialists within the community to get help in achieving care plan goals
