Requirements

  • Bachelor's degree in Nursing, Healthcare Administration, or a related field is preferred.
  • Current licensure as a Registered Nurse (RN).
  • Minimum of two years of experience in long-term care or skilled nursing facility is preferred.
  • Strong knowledge of MDS 3.0 guidelines, federal and state regulatory requirements.
  • Excellent organizational, analytical, and problem-solving skills.
  • Effective communication and interpersonal skills for engaging with residents, families, and staff.
  • Proficiency in electronic health records (EHR) systems and Microsoft Office Suite.

Responsibilities

  • Oversee the completion of MDS assessments and ensure timely submission to state and federal agencies.
  • Coordinate interdisciplinary team meetings to develop and review individualized care plans for residents.
  • Conduct staff training on MDS procedures and regulatory compliance to ensure all standards are met.
  • Review and analyze data from MDS assessments to identify trends and improvement opportunities in care delivery.
  • Maintain accurate records and documentation to support compliance with regulatory standards.
  • Collaborate with healthcare professionals to ensure that assessments align with residents' needs and preferences.
  • Monitor outcomes of care plans and make necessary adjustments to improve quality of care and resident satisfaction.
