Requirements

Active RN license (required).
Proven experience as an MDS Coordinator in skilled nursing or long-term care. (minimum of 2 year MDS experience required)
Current RN license in the state of Wisconsin.
Experience preparing for and navigating state surveys.
Exceptional attention to detail and strong organizational skills.
Ability to work independently while collaborating effectively with interdisciplinary teams.
Strong communication skills and confidence in clinical decision-making.

Responsibilities

Oversee and coordinate the MDS process for resident assessments and care plans.
Coordinate and complete accurate, timely MDS assessments
Ensure compliance with all federal and state regulations.
Collaborate with the interdisciplinary team to develop and update comprehensive, resident-centered care plans.
Monitor clinical documentation to ensure accuracy and optimal reimbursement.
Manage assessment schedules and submission deadlines with organizational precision.
Support survey readiness and participate in quality improvement initiatives.
Provide education and guidance to nursing staff regarding documentation standards.
Serve as a clinical resource for reimbursement processes and regulatory compliance.
Collaborate with interdisciplinary team members to implement comprehensive care plans.
