Requirements

  • Licensed nurse (RN preferred; LVN considered with strong experience)
  • Experience with MDS coordination in a skilled nursing facility
  • Knowledge of PDPM, CMS regulations, and care planning
  • Strong organization, communication, and time management skills

Nice To Haves

  • Infection prevention experience is a plus (training provided as needed)
  • PointClickCare (PCC) experience preferred

Responsibilities

  • Coordinate and complete accurate, timely MDS assessments in accordance with CMS guidelines
  • Ensure proper documentation to support PDPM reimbursement and clinical outcomes
  • Collaborate with the interdisciplinary team (IDT) to develop and update care plans
  • Track MDS schedules and maintain compliance with submission timelines
  • Support audits and ensure chart accuracy and strong clinical documentation
  • Support and maintain the facility’s Infection Prevention & Control Program
  • Monitor infection trends and assist with investigations and reporting
  • Provide staff education on infection control policies, PPE, and prevention practices
  • Ensure compliance with CDC, CMS, and CDPH guidelines
  • Assist with prevention strategies to protect residents and staff
