About The Position

Saint Therese is seeking an experienced MDS/RAI Coordinator to support exceptional, person-centered care through accurate assessment and care planning in a faith-based, nonprofit senior community. This role is ideal for a mission-centered RN or LPN with deep knowledge of the MDS/RAI process, Case Mix, and Medicare/managed care, who enjoys combining clinical expertise with data accuracy and interdisciplinary collaboration. Requirements

  • Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in good standing with the Minnesota Board of Nursing.
  • Minimum three (3) years of long-term care experience.
  • Demonstrated experience with: MDS/RAI process and Case Mix.
  • PPS, Medicare, HMOs, managed care, and third-party payors.
  • Care planning in a long-term care environment.
  • Strong written and verbal communication skills.
  • Proficiency with computer systems and software (Microsoft Office, facility/EHR software, ROX, Excel).
  • Ability to work both independently and collaboratively, with a high level of professionalism, respect, and commitment to quality care.

Nice To Haves

  • Certification in Gerontology preferred.

Responsibilities

  • Coordinate and complete the full Resident Assessment Instrument (RAI) / MDS process for assigned residents.
  • Ensure timely, accurate assessments in compliance with State and Federal regulations.
  • Conduct resident interviews, participate in resident care conferences, and attend discharge meetings as needed.
  • Apply extensive knowledge of Case Mix, PPS, Medicare, HMOs, managed care, and third-party payors.
  • Utilize appropriate ICD-9/ICD-10 diagnosis coding to support accurate reimbursement and care planning.
  • Review charts, admission information, and documentation; identify areas of concern and escalate to the appropriate Clinical Coordinator.
  • Partner with nursing and interdisciplinary team members to identify and address quality of care issues.
  • Use assessment data to support individualized care plans that reflect resident needs and preferences.
  • Contribute to continuous improvement efforts aimed at regulatory readiness, quality outcomes, and resident satisfaction.
  • Maintain accurate records and reports using Microsoft Office, facility/EHR software, ROX, and Excel.
  • Manage time effectively and prioritize assessments to meet regulatory and organizational timelines.
  • Support consistent workflows and documentation standards within the clinical team.
