The Medical Assistant reports to the Director of Clinical Administration and supports the Chronic Care Management Program by providing both clinical and administrative assistance to the care delivery team. This role is integral to ensuring coordinated, high-quality, and cost-effective patient care by supporting care teams, facilitating communication, managing documentation, and assisting with care transitions across healthcare settings.

Requirements

  • Must be Bilingual in English and Spanish.
  • High School Diploma or equivalent
  • Current certification or registration (CMA, RMA, CCMA, or equivalent).
  • Basic Life Support (BLS) certification
  • 1+ years of experience in a clinical, healthcare, primary care, or outpatient clinical setting.
  • Familiarity with EMR systems and basic clinical documentation.
  • Proficiency with Microsoft Office Suite, including Outlook, Word, Excel, and Teams.
  • Customer Service experience
  • Ability to pass background check upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/trace search (LEIE), education report (highest degree obtained), civil upper and lower search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen.
  • High-speed internet service (cable or fiber optic) with minimum download Speed of 20 Mbps, Upload Speed of 5 Mbps, and Maximum Latency of 100 milliseconds (must be installed before starting) required.
  • Frequent use of computer and phone systems
  • Must be able to constantly remain in a stationary, sitting position, communicate and exchange information with others, inspect information, perform repetitive motions with arms and fingers, interpret data, problem solve, make decisions, organize and plan, and maintain a positive and professional attitude in all situations.
  • A quiet workspace with minimal background noise for calls.

Responsibilities

  • Serve as a primary point of contact for patients by conducting intake and confirmation calls, communicating provider‑reviewed results and care plans, and supporting initial visit preparation.
  • Coordinate scheduling and rescheduling for in‑person and telehealth visits, follow up on missed appointments, and ensure continuity when provider availability changes.
  • Track patient admissions, discharges, and activity across hospitals, skilled nursing facilities, and other care settings; document updates in the EMR and notify the care team.
  • Support transitions of care by obtaining and uploading medical records, coordinating respite or long‑term care stays, and scheduling return‑to‑residence visits as directed.
  • Provide day‑to‑day operational and clinical support to primary care teams, including managing the clinical inbox, processing medication refills, prior authorizations, and insurance‑related forms.
  • Collaborate with multidisciplinary teams through meetings and improvement initiatives, escalating clinical concerns appropriately to RNs or providers and keeping PCPs and specialists informed of care plan changes.
