About The Position

The Medical Case Manager I (MCM) will serve as part of UofL Ryan White Kentucky Care Coordinator Program (KCCP) case management team. The MCM will provide overall case management, support and advocacy for UofL Ryan White eligible clients who adhere to stated program service and enrollment requirements. The goal of the MCM is to aid in removing barriers to care access and to drive improved client outcomes across the KIPDA Region as part of UofL Ryan White program. Thirteen (13) Medical Case Manager I positions are available.

Requirements

  • Bachelor's degree in Social Work, Sociology, Psychology, Nursing or related field or the equivalent combination of education and experience.

Nice To Haves

  • 2 years of experience working within HIV care services community.
  • Proficient in a second language such as Spanish, Somali or Nepalese.

Responsibilities

  • Maintain an active caseload based on state-mandated caseload standards by providing non-medical case management, including counseling to support readiness for and adherence to complex HIV/AIDS treatments.
  • Conduct initial acuity assessments to identify client service needs and develop comprehensive Service/Care Plans; coordinate appropriate services to achieve identified goals.
  • Complete required eligibility documentation every six months, including verification of HIV status/diagnosis, residency, and income.
  • Reassess and update Service/Care Plans at least every six months or as needed throughout the eligibility period.
  • Document all case management activities in the CareWare database, including services provided, type and frequency of encounters, and communication details.
  • Assist clients in accessing eligible public and private programs and services.
  • Coordinate and follow up on medical appointments, laboratory tests, and monitor viral load suppression.
  • Provide client education on disease transmission, prevention methods, and healthy lifestyle practices throughout ongoing case management.
  • Participate in ongoing HIV-related training, including Ryan White Part B conferences and seminars.
  • Engage in clinical quality management (CQM) activities as assigned.
  • Perform other duties as assigned.
