About The Position

VitalCore Health Strategies (VCHS), an industry leader in Correctional Health Care has an opening for a Full-Time Medical
Discharge Planner at MCI-Norfolk & Pondville Correctional Center

The Medical Discharge Planner provides access to community programs, ensure continuity of care following release into the community, advocate for patient needs, and act as liaisons between the multiple systems impacting the lives of offenders with mental health illnesses, co-occurring substance use disorders, and chronic medical illnesses.

Requirements

• A bachelor's degree in Mental Health or related field
• Case Management experience
• Familiarity with medical/MH terminology/diagnosis should be required
• Knowledge of accessing services in community

Nice To Haves

• Applicants with experience in a correctional health care setting will be given preference.
• Licensing preferred, not required.
• Baseline understanding the medical parole process preferred, not required.
