Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. As a Medical Social Worker, you won’t just be “filling a role” — you’ll be a lifeline of support, helping patients and their loved ones navigate emotional, social, and practical challenges with warmth and understanding. Requirements
- Master’s Degree in Social Work (MSW)
- A heart for service, empathy, and compassion
- Strong communication and listening skills
- A collaborative, team-focused spirit
Nice To Haves
- Experience in hospice, healthcare, or counseling preferred
Responsibilities
- Provide emotional support, counseling, and education to patients and families
- Guide families through difficult decisions and connect them with community resources
- Advocate for patient needs as part of an interdisciplinary care team
- Support patients’ wishes, dignity, and spiritual well-being
- Help ensure that no patient or family ever feels alone in their hospice journey
