About The Position

The Medication Intake Technician obtains medication and allergy histories at time of patient admission to hospital. In this role information is gathered by interviewing the patient, patient family, and other available resources such as the patient's pharmacy. Assists Pharmacists and other clinical staff by identifying any discrepancies and maintaining efficient flow of day to day operations for the Pharmacy Department.

Requirements

  • High School Diploma/Equivalent
  • 0 - 1 year Med Intake/Pharmacy Technician
  • Registered Pharmacy Technician (RPT) in Florida OR Pharmacist Intern

Nice To Haves

  • One to three years of pharmacy technician experience in a community pharmacy (retail). At least 6 months of pharmacy experience.
  • Certified by Pharmacy Technician Certification Board (PTCB) or Exam for the Certification of Pharmacy Technicians (ExCPT)

Responsibilities

  • Obtains patient medication history upon admission enabling the continuity of appropriate drug therapy and ensures a seamless transition of care at time of patient discharge from hospital.
  • Obtains relevant patient information utilizing a variety of sources (i.e. prescription pill bottles, outside pharmacy, primary care providers, medication administration records, drug databases, etc), then verifies through patient and/or family interviews.
  • Identifies and records in electronic medical record accurate name, dose and directions, and time of last dose taken for all prescription medications, over the counter drugs and herbals/supplements.
  • Interacts with and provides medication history and other related patient information to nurses, physicians, pharmacists and other medical professionals as appropriate.
  • Communicates clearly and effectively both in person and on the phone with patients’ medication history and treatment needs to a variety of audiences including patients, patient care providers, physicians, pharmacists, and nursing staff.
  • Identifies and reports discrepancies in the patient’s medical record including: conflicting allergy information, changes or duplication of medications entered on previous admissions. Assists pharmacists and nurses to resolve discrepancies.
  • Assists pharmacists and director of pharmacy in all functions to maintain smooth operation of the department.
