Overview The Acute and Urgent Services Coordinator is a staff member embedded within the Community Behavioral Health Center (CBHC) Mobile Crisis Intervention (MCI) team, who also works closely with the Community Crisis Stabilization (CCS) program directors, Care Coordination team, and Referral and Intake Department to ensure and facilitate coordination, engagement, and warm hand-offs across the program continuum. Responsibilities Job Duties and Responsibilities: The essential job duties/responsibilities of the position include but are not limited to the information listed below: Ensures a trauma-informed environment that promotes safety and recovery. Participates in triage functions and coordinate and organize/assign follow up tasks, ensuring continuity of care and follow up within the CBHC. Facilitates warm hand-offs to CCS and Outpatient [OP] (urgent or routine therapy and medications) by working closely with directors/managers/supervisors in addition to the front-line staff. Collaborates around tracking client flow through the CBHC service to increase accessibility; and problem-solve barriers to engagement and connection. Participates in client transitions via intake screening and assessment and determination of appropriate programming within the CBHC (e.g., may participate in client intake process and support referral to care coordination). Provides follow up calls and interventions as instructed by the Director of Acute Care and Urgent Outpatient Services and as outlined in the client’s treatment/care plan/Discharge and Wellness Plan (this may include referrals to services or appropriate level of care, coordinating transportation, ensuring warm transfers, connecting with care coordination, helping to orient to Community Crisis Stabilization and/or other treatment settings, etc.) Provides crisis intervention services including crisis case management and short-term crisis counseling as follow-up from initial evaluation. Maintains linkages with specific community services and provides problem resolution. Offers recommendations to clients regarding service options and support in accessing. Develops and maintains a thorough knowledge of managed care and health insurance systems; provide other clinicians with managed care information and resources pertinent to the crises being managed. Engages in insurance verification process and assists with billing tasks. Maintains necessary documentation and records. Actively participates in supervision and staff meetings.
