About The Position

Wound Care Provider (NP/PA) | Facility-Based Rounds | Grand Rapids NIVA Health is hiring a Wound Care Provider in the Grand Rapids area to deliver advanced wound care through a facility-based rounding model. You’ll work in partnered clinical settings such as skilled nursing facilities, rehab centers, and assisted living communities, collaborating with on-site care teams to improve healing outcomes and patient quality of life. This role is a great fit for a clinician who enjoys clinical autonomy, a consistent workflow, and working alongside established facility staff.

Requirements

Active Nurse Practitioner/Physician Assistant (NP/PA) license.
Valid DEA license.
Strong independence, communication, and organizational skills.
A passion for patient-centered care.
Reliable transportation for local travel between facilities.

Nice To Haves

Clinical experience in wound care, SNF/rehab, home health, or related settings is a plus.
Strong clinical judgment and communication skills.
Wound Care Certification (preferred, with CEUs provided if needed within 90 days of hire).
Experience with wound documentation and treatment planning

Responsibilities

Provide comprehensive wound assessments and treatment plans
Deliver advanced wound care services, including dressing selection, treatment protocols, and ongoing monitoring
Perform debridement and other procedures within your scope and credentialing (training and support available)
Coordinate care with facility nursing teams and leadership
Document visits clearly and efficiently in our system
Communicate recommendations and follow-up plans with the care team
