Requirements

  • Registered Nurse (RN) license in good standing. Possess license in state of residence.
  • Minimum 5 years of experience in case management or related clinical role.
  • Proven ability to analyze data and develop strategic care plans.
  • Strong communication and collaboration skills.
  • If you will be working at home occasionally or permanently, the internet connection must be obtained through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload.

Nice To Haves

  • Bachelor’s degree in Nursing.
  • Experience with healthcare analytics and population health management.
  • Certification in Case Management (CCM or equivalent).

Responsibilities

  • Lead strategic initiatives to improve patient care coordination and optimize resource utilization.
  • Analyze complex clinical data to identify trends and recommend actionable solutions that enhance quality of care.
  • Collaborate with cross-functional teams to design and implement innovative case management programs.
  • Drive continuous improvement by monitoring outcomes and adjusting strategies to meet evolving patient needs.
  • Serve as a trusted advisor, providing guidance and mentorship to team members to foster professional growth.

  • For this position, we anticipate offering an annual salary of 77,500 - 129,100 USD / yearly, depending on relevant factors, including experience and geographic location.
