Acts as a liaison between providers, departments, other healthcare professionals and schedulers to coordinate testing, scheduling, referrals etc. Develops and implements strategic interventions across the system to improve patient access and navigation.

Requirements

  • Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).
  • State criminal background check and Federal (if applicable), as required for regulated areas
  • Obtain certification in Basic Life Support within 30 days of hire date.
  • Two years’ experience as an RN OR five years’ experience as an LPN required.
  • Strong working knowledge of scheduling workflows and referral processes.
  • Ability to troubleshoot complex access and coordination issues.
  • Excellent communication and problem-solving skills.
  • Proficiency in Epic or similar EHR platforms.
  • High attention to detail and accuracy.
  • Ability to work collaboratively with diverse clinical and administrative teams.
  • Strong organizational and multitasking abilities.
  • Ability to identify system-level barriers and propose actionable solutions.

Nice To Haves

  • Bachelor's Degree in nursing(BSN).
  • Experience in outpatient clinical setting.
  • Proficient with Epic Cadence/Scheduling.

Responsibilities

  • Attends conferences or educational opportunities to ensure up to date knowledge.
  • Act as point of escalation for providers, providing triaging and medical advice where applicable.
  • Coordinate scheduling, testing, and referrals across multiple departments to ensure patients move efficiently through their care pathway.
  • Monitors and manages patient’s appointments, testing and labs necessary for the patient population.
  • Partners with specialty leaders and enterprise resources to identify areas of opportunity for improvement of access and navigation
  • Develops and implements strategic interventions across the system to improve patient access and navigation
  • Review patient charts to identify outstanding testing, labs, or clinical prerequisites and ensure completion before appointments.
  • Facilitate accurate and complete referrals, ensuring required documentation and clinical information are in place before scheduling.
  • Document coordination activities and communication in the EHR, using Epic tools and work queues to track needs and progress.
  • Participate in relevant meetings or workgroups focused on patient access, navigation, and workflow optimization.
