About The Position

Stamford Health has an opportunity for a full-time Certified Diabetes Educator (CDE), RN in Stamford! The Registered Nurse will work in the outpatient Diabetes Education Program as a Certified Diabetes Educator (CDE). Nurse will be a member of the education team and will perform patient assessments as well as develop, instruct and evaluate both individual and group sessions in accordance with the current National Standards for Diabetes Self-Management Education. The nurse will teach evidence based approved curriculum and participate in maintaining American Diabetes Association Recognition status and actively participate on the Diabetes Advisory Committee. This team member will be knowledgeable in insulin pump and continuous glucose monitoring training and will obtain manufacturers product certification as needed. Schedule: Monday-Friday 8:00 am - 4:00 pm

Requirements

  • BSN required. Current CT. Registered Nurse License.
  • Minimum 3-5 years of recent clinical experience with evidence of diabetes related teaching. CDE (Certified Diabetes Educator) required. Credential required or obtained within 2 years of hire.
  • Current certification in Insulin Pump Training and Continuous Glucose Monitoring is required or to be obtained within 6 months of hire.

Responsibilities

  • Performs individual assessments, instruction, counseling / support, goal setting, evaluation, and follow up in accordance with the national standards
  • Utilizes age specific guidelines as well as religious/ cultural practices while delivering patient and family centered care.
  • Assess the physical, psychosocial and learning needs and status of our patients on an ongoing basis.
  • Develops and communicates an instructional plan of care reflecting identified patient needs, interventions to achieve desired outcomes, and input from the patient, family and healthcare team.
  • Evaluates patient outcomes, effectiveness of plan of care.
  • Revises plan of care as needed and makes referrals where indicated.
  • Utilizes American Diabetes Association approved curriculum / tools to plan and deliver instructional content including problem solving and interactive multidisciplinary activities.
  • Applies and engages in evidence based quality improvement and processes specific to SH to achieve a more effective program.
  • Uses clinical and behavioral outcome measures to gauge effectiveness of practice and program.
  • Completes 15 hours of annual continuing education hours related to Diabetes Care.This is required for maintaining CDE credential..
  • Communicates effectively with other team members.
  • Supports hospital-wide and unit based Nursing Shared Governance framework and initiatives.
  • Participates in ongoing Community Health Outreach activities.
  • Participates in peer review /feedback activities, self- appraisal, and goal setting to enable professional development.
  • Demonstrates professional work behavior by following Service Standards, Success Factors and Nursing Code of Ethics.
  • Competently practices within the scope and standards of practice for a licensed nurse. Follows all state, federal and professional organization guidelines.
