Brigham & Women's Faulkner Community Physicians, Primary Care West Roxbury & Hyde Park, MA Full time/ 40 Hours Brigham & Women's Faulkner Community Physicians (FCP) has been partnering with the community for over 20 years. We are an established, teaching primary care practice of Brigham and Women's Hospital dedicated to providing multi-disciplinary, exceptional care to our community. We are a diverse and dynamic team focused on adult preventative medicine and chronic disease management. The practice is located in 2 sites in West Roxbury and Hyde Park. On-site laboratory services are available (by appointment) for our patients, along with a patient portal (Patient Gateway) which can be used to request appointments, request referrals, view lab results, and interact with staff regarding non-urgent matters. We also have appointment availability with our social worker. Our primary care physicians are board certified in internal medicine. Physicians in the Division of General Internal Medicine & Primary Care serve as faculty members of Harvard Medical School and are committed to teaching and research. The Nurse Manager is a key membership of the Triad practice leadership team, with direct reporting accountability for registered nurses, licensed practical nurses, medical assistants and MA Leads. GENERAL OVERVIEW: The RN Nurse Manager (Flow Manager) is valued member of the clinical leadership team. The primary responsibility of the Nurse Manager is to coordinate and expedite the clinical operations of a multi-disciplinary team, ensuring an exceptional patient experience. The Nurse Manager reacts to the immediate operational needs of the practice, supervising and directing other members of the interdisciplinary team to further the coordination and advancement of care of patients. Develops strong collaborative relationships across roles, disciplines and departments with a focus that ensures the needs of patients are addressed. The Nurse Manager is accountable for facilitating an appropriate response to the access needs of patients in alignment with practice based resources.
