Short Description Facilitates coordination for reconstructive and general plastic surgery needs collaborating with plastic surgeons and surgical teams to ensure appropriate referrals, pre-operative preparation, and continuity of care for patients requiring complex wound closure, soft tissue reconstruction, or post-oncologic reconstructive procedures. Coordinates the continuum of care for breast cancer patients undergoing surgical treatment, facilitating seamless collaboration between breast surgeons and plastic surgeons to support timely reconstruction planning and optimal clinical outcomes. Serves as the central point of contact for patients, guiding them through surgical decision-making, reconstruction options, and treatment timelines while ensuring clear communication among the multidisciplinary care team. Streamlines pre-operative and post-operative coordination, including scheduling, education, and preparation for procedures to improve patient experience and reduce delays in care. Provides patient education and navigation support, helping patients understand surgical pathways, choices, and recovery expectations while addressing barriers to care.

Requirements

  • Minimum of 3 years surgery nursing and/or care coordination experience and background a plus.
  • BSN required.
  • Current NJ-RN License (Registered Nurse), BLS and ACLS required.
