About The Position

Spurwink is looking for a collaborative, engaging, and driven Registered Nurse (RN) with unencumbered Maine licensure to join our interdisciplinary team serving adults and children with a variety of mental health needs, including serious mental illness and substance use disorders. This position will work across programs to address primary prevention, health promotion, and population health across regional services by: Providing nursing consultation to staff, interpreting health data, and helping to integrate health and wellness goals into client treatment plans. Nursing care includes a focus on community outreach, community integration, education, and time-limited patient-centered care aiming towards promoting independence in managing and maintaining optimal health. Improving communication and collaboration with all care providers across the healthcare spectrum. Conduct home visits as appropriate for individuals and families in care coordination, providing health education, consultation, and collaborative support to further their mental and physical health goals. This is a full-time position with no nights, weekends, holidays, or on-call work required. The pay is $32.00-$38.00 per hour depending on experience.

Requirements

  • Maine licensure as a Registered Nurse (RN).
  • Three years of post-licensure experience in community health nursing, family nursing, pediatric nursing, hospital, or psychiatric nursing preferred.
  • Direct service experience working with individuals with severe and persistent mental illness, serious emotional disturbance, and developmental disabilities is preferred.
  • Must have the ability to exercise professional judgment in making decisions, communicate effectively with service recipients, family members, case managers, administrators, community providers, and other selected personnel.
  • Must possess a high level of organizational and time management skills.
  • Must be computer literate and able to navigate an electronic medical health record, utilize HealthInfoNet, generate PowerPoint presentations, and present basic data summaries to management.
  • Knowledge of psychosocial, mental health, and vocational assessments, counseling theory such as MI, CBT, and DBT, physical and psychological effects of mental illness and substance abuse preferred.

Responsibilities

  • Effectively engage, assess, monitor, educate, coordinate, and provide interventions for participants and families to improve quality of care and participant health outcomes.
  • Provide participant, family/caregiver teaching and education about health and wellness, safety factors, self-management skills, medications, self-monitoring, action plans, long-term planning, when and how to access care.
  • Conduct in-person visits with participants in the community, in participants' homes, hospitals and care facilities, physician offices, based on the participants' needs.
  • Facilitate group treatment and education interventions.
  • Participates as a member of a multidisciplinary team, by providing health information related to participant medical history and needs, medications, co-morbidities, immunizations, dietary needs, and the behavioral, social, psychological, and environmental determinants of health.
  • Assist treatment team with development, implementation and ongoing monitoring of ISPs, and document ongoing nursing care and interventions in the electronic health record and elsewhere.
  • Maintain regular contact, effectively collaborate with, and facilitate referrals to community providers to advocate on behalf of, and improve the care for, participants.
  • Adhere to procedures and practices outlined in Health Home MOUs.
  • Maintain expectations related to caseload size, billing, and attestation requirements.
  • Maintain required records and documentation.
  • Collect, track, and submit participant related data to support evaluation of participant outcomes, program goals and performance.
  • Serve as a resource for staff, colleagues, and external members of a participant's care team.
  • Provide back-up for Medical Assistant to coordinate delivery of samples from drug reps, Enroll/ re-enroll patients in the patient assistance programs, administer antipsychotic injections and oral medications, pack medication caddies, and assist with appointment scheduling.
