Some highlights of this opportunity include: · Congestive Heart Failure Nurse Practitioners autonomously manage a patient panel of recently discharged and chronic acute exacerbation patients in an outpatient ambulatory clinic setting · Responsibilities include remote monitoring of heart failure patients, managing LVAD patients, collaborating with cardiologists, and oversight and referring out for comorbidities · Extremely competitive per diem compensation · Per diem - Available shifts are between M-F 8am-4pm · The ability to work with complex patients in an autonomous ambulatory setting St. Vincent's cardiac program is recognized regionally and nationally for innovation and outstanding clinical results. Requirements

  • Eligibility to be licensed as an Advanced Practice Registered Nurse (APRN) in Connecticut is required.
  • Eligibility to be certified as a Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), Adult Gerontology Primary Care Nurse Practitioner (AGPCNP), Acute Care Nurse Practitioner (ACNP), or Adult-Gerontology Acute Care Nurse Practitioner (AGACNP) is required.
  • Experience in practicing as an Advanced Practitioner is required.

Nice To Haves

  • Congestive Heart Failure experience is highly preferred.
