About The Position

We are seeking actively licensed Nurse Practitioners for 1099 Independent Contractor opportunities to perform HCC Wellness Assessments. This flexible role provides an excellent way to earn supplemental income while delivering high-quality preventive care to senior patients.

Requirements

Active California Nurse Practitioner License
Active California RN License
Current BLS certification
Valid California Driver's License and proof of insurance.
HCC experience
Demonstrated proficiency in the use of Microsoft Office, Word, Excel, and other applications as required
Excellent observational, verbal, and written communication skills
Interpersonal skills necessary to work productively with patients, families, nursing staff, and other departments

Nice To Haves

Bilingual, Spanish speaking helpful
Three to five years of experience as a Nurse Practitioner

Responsibilities

Provides anticipatory guidance, screening, and health education to patients to promote and maintain health.
Provides annual health assessments for senior patients in a variety of settings, such as in our Senior Wellness Clinic, in various provider offices, and at the patient's home.
Assesses senior members for preventative interventions such as safety, fall risk, depression screen, and urinary concerns.
Evaluates the chronic conditions of senior structures individuals, and/or families or groups about health or defined health problems.
Provides education to patients and family members based on screenings, assessments, and the patient's chronic conditions
Plans appropriate care based on screening and chronic care guidelines.
Plans appropriate care utilizing community resources and acts as a liaison by relaying information necessary for continuity of care to other health professionals and agencies.
Flexible with the setting of the evaluations based on the company's needs.
Promotes guest relations; anticipates and responds to the needs of others; assists in a courteous, quick manner; treats others with care and respect, while maintaining privacy, confidentiality, and dignity.
Maintains a professional appearance and a clean, safe environment. Participates with the company's Quality and Coding departments to ensure the requirements established by the Centers for Medicare and Medicaid Services (CMS) are fulfilled for each senior patient.
Provides clinical services by managing patients in the home or clinic.
