About The Position

Are you a Nurse Practitioner looking for a flexible, autonomous role focused on preventive care instead of high-volume clinic visits? We are hiring Full-Time Nurse Practitioners to complete Comprehensive Health Assessments (CHA) and Health Risk Assessments (HRA) for Medicare and Medicaid members in their homes and residential settings. This role focuses on preventive care, early detection, and closing care gaps — not managing ongoing treatment plans or maintaining a patient panel. During a 60-minute in-home visit, you will: Perform comprehensive adult and geriatric assessments Review medical history and complete medication reconciliation Identify chronic conditions and care gaps Conduct preventive screenings and point-of-care testing (as applicable) Provide patient education and wellness counseling Document findings accurately in EMR Collaborate with PCPs and care teams when needed This role does NOT require managing ongoing primary care panels. Requirements

  • Master’s Degree or higher from an accredited NP program
  • Active NP license in the state of practice
  • Board Certification (AANP, ANCC, AACN)
  • BLS certification
  • 1+ year NP experience preferred (new grads welcome to apply)
  • Valid driver’s license & auto insurance
  • Comfortable with EMR documentation
