About the position
Theoria Medical is seeking a dedicated provider to manage clinical care at skilled nursing facilities (SNFs). This full-time position offers a unique opportunity to deliver high-quality, patient-centered primary care services. The provider will be responsible for daily rounding, prescribing medications, and ensuring timely documentation of patient encounters. The role emphasizes meaningful connections with patients and staff, supported by a community of healthcare professionals. The position is in-person, with a structured shift from 7 AM to 1 PM, Monday through Friday, allowing for a balanced work-life schedule.
Responsibilities
- Rounding and managing clinical care of the population at assigned SNF.
- Prescribing medications and treatments for residents at SNFs.
- Providing high-quality, patient-centered primary care services.
- Timely documentation of all patient care encounters.
- Available for phone/secure messaging consultations with facility nursing staff.
- Completing documentation in Theoria’s proprietary EHR system, ChartEasy™.
- Ensuring timely attendance and reliable transportation for in-person shifts.
Requirements
- Valid medical licensure.
- Experience in primary care or related field.
- Ability to perform essential functions satisfactorily, with or without reasonable accommodation.
Nice-to-haves
- Experience with EHR systems such as PointClickCare, MatrixCare, or American HealthTech.
- Strong communication skills for patient and staff interactions.
