About The Position

The Cardiac Electrophysiology (EP) Nurse Practitioner (NP) will practice in three settings: inpatient, outpatient/ambulatory, and procedural settings. The NP will be involved in all aspects of care with the primary goal of ensuring the effective delivery of care to patients. Inpatient and peri-procedural care - The Nurse Practitioner will be responsible for obtaining and coordinating the following general items: history and physical, laboratory results, charting of clinical findings, registry documentation, orders, sedation assessment, nursing instructions, consent, and other required elements. The NP, in collaboration with the Laboratory Flow Managers, may be asked to help coordinate admissions and transfers to the hospital. The NP will perform pre-procedure assessments in the recovery room/holding area or outpatient setting as needed. - The Nurse Practitioner (NP) will be knowledgeable about the Cardiac Electrophysiology Laboratory scheduling, procedure room turnover and availability of resources. They will be called upon to manage patients after their procedures (as appropriate and in collaboration with the consult Fellow) while they await transfer to patient floor or discharge. The NP will assist in family liaison. - Following procedures, the Nurse Practitioner (NP) will manage vascular access, monitor and manage arrhythmias, pacing, drug therapy, O2 therapy, and other non-cardiac medications, under the supervision of the attending physician. The NP will assist in the evaluation and management of clinical events such as fever, altered hemodynamics, CHF, pulmonary complications, abrupt vessel closure, arrhythmias, access site complications and others. The NP will assist in transfers to and from the intensive care units. - The Nurse Practitioner (NP) will perform patient education regarding discharge including preventive care and risk factor reduction. They will write discharge orders, prescriptions and complete discharge summaries in collaboration with the attending physician. The NP will act as a liaison between the attending cardiologist and family members and will schedule follow-up care as necessary, including VNA if needed. Ambulatory care - The Nurse Practitioner (NP) will collaborate with the team in preparing patients for admissions/procedures. Educate patients regarding a variety of electrophysiology treatments and procedures. Works collaboratively with nursing/clinic staff to assist in the care of patients. This includes, but is not limited to: symptom management, evaluation and management, prescription writing, ordering and interpreting diagnostic studies with review by attending MD, patient calls and questions related to electrophysiology clinical care. The NP will provide care via in person clinic visits, virtual clinic visits, and telehealth. Procedural care - The Nurse Practitioner (NP) will be responsible for many aspects of care under the supervision of the attending physician, including but not limited to: cardioversions, suturing, cardiac subcutaneous monitor implant/explant, first assisting during diagnostic EP studies/ablations, device implantation and research procedures performed in the EP laboratory, OR and other clinical areas, as well as fluid and electrolyte management, nutrition, pain management, transfusion therapy, anticoagulation management, education, and post-discharge planning. General/Working Conditions - The Nurse Practitioner (NP) participates in departmental and organizational committees and programs as appropriate and attends all required meetings, in-services, and professional trainings. Remains actively involved in continuing education with a commitment to self and departmental growth. Identifies areas for professional growth. Formulates professional goals, objectives and methods for accomplishing these. Supports initiatives within BWH, the Department of Medicine and the Cardiovascular Division. - Work settings may include ambulatory clinics and treatment rooms, inpatient areas, intensive care units, operating rooms, and/or procedural areas. There may be loud noises, noxious odors, and temperature extremes. A large portion of the workday may be spent either standing or walking and the position may require heavy lifting of equipment and patients. - The Nurse Practitioner (NP) may be exposed to radiation, potentially infectious bodily fluids, and sharp instruments such as needles and scalpels. Knowledge of Universal Precautions and proper use of surgical sharp instruments may be required. The consequences of contamination from bodily fluids or from being stuck with a sharp instrument include the possibility of physical harm to the employee or others. BWH is a fast paced, dynamic environment with a very large and complex patient volume; exposure to stress in the workplace is expected. Job Summary The Nurse Practitioner is an advanced practice registered nurse who has acquired specialized knowledge and skills through completion of a nurse practitioner program. The practitioner has the credentials to function in an expanded nursing role and is responsible for the daily management of selected patients. The practitioner has responsibility for the assessment and management of patients, including diagnostic and therapeutic interventions, and the continuous implementation and evaluation of an appropriate plan of care. The Nurse Practitioner works with their collaborating physician in accordance with mutually developed practice guidelines, members of the care team, and is responsible to the Nursing Director or Executive Nursing Director for the area for issues specific to the discipline of nursing. Satisfactory completion of an approved formal education program that has as its objective the preparation of professional nurses to practice as nurse practitioners. ANA or other appropriate nursing certification in the area of practice and authorization by the Massachusetts Board of Registration in Nursing to practice in the expanded role. DEA for prescription writing.

Requirements

  • Master's in Nursing (MSN) (or) Doctor of Nursing Practice (DNP) (or) PhD Degree in Nursing required.
  • Completion of a Master’s or post Masters’ degree in Adult program accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC).
  • Satisfactory completion of an approved formal education program which has as its objective the preparation of professional nurses to practice as nurse practitioner.
  • Current Adult/Gerontology (AGNP) or Family Nurse Practitioner (FNP) Certification by an accredited APRN certifying body required.
  • DEA registration required and current APRN License to practice in Massachusetts required.
  • Minimum 5 years’ nurse practitioner experience caring for a cardiac electrophysiology focused patient population required.
  • Cardiac Implantable Electronic Device (CIED) interrogation and programming experience required.
  • Expert clinical judgment skills to function in the independent role.
  • Excellent interpersonal relationship and communication skills in order to collaborate with patients’ families, staff and community health workers.
  • Ability to work with frequent stress resulting from busy clinical settings and changing organizational climate.
  • Ability to be self-directive and demonstrate initiative in addressing responsibilities of position.
  • Ability to utilize appropriate problem solving and conflict resolution skills.

Responsibilities

  • Provides direct care, counseling, and teaching to a select patient population.
  • Performs physical examinations and provides preventive health measures appropriate to patient needs.
  • Orders, interprets, and evaluates appropriate lab and diagnostic tests to assess patients’ clinical problems and health care needs.
  • Arranges for appropriate plan of care and follow-up based on outcome of diagnostic, lab, and physical assessment findings.
  • Performs laboratory tests appropriate for specific patient population.
  • Orders medications and writes prescriptions according to organizational and regulatory policies and procedures.

  • Sets priorities for appropriate and efficient management of patient care that reflects cost effective management of time, available human resources, supplies and equipment.

  • Assists in the development, implementation and revision of nurse practitioner guidelines and monitoring requirements for prescription writing in collaboration with their designated physician and Nursing Director or Executive Nursing Director.

  • Collaborates with the Nursing Director/Practice Manager/Executive Nursing Director in the recommendation of appropriate staffing patterns for their clinical areas.
  • Participates in selection process of new employees in group or team practice as requested.
  • Provides input into evaluation of other staff as appropriate.
  • Acts as preceptor for nursing students and staff as requested.
  • Participates in orientation of new staff.
  • Participates in Departmental and organization continuing education programs.
  • Identifies own learning needs and updates clinical knowledge and skills through formal and informal education and review of current literature.
  • Educates lay and/or professional groups through lectures, papers, publications, and educational materials.
  • Develops annual objectives and reviews with Nursing Director or Executive Nursing Director and collaborating M.D. at time of performance review and at appropriate intervals. Completes annual credentialing process through the Nursing Department credentialing program.
