About The Position
We’re looking for a Nurse Practitioner who loves primary care and values building real relationships with patients. Our busy, well-established primary care practice in the Farmington Valley is growing, and we’re excited to add a compassionate, team-oriented APRN to our group. We pride ourselves on providing thoughtful, patient-centered care in a supportive and collaborative environment. This is an outpatient position with a strong support team and an established patient base. You’ll have the opportunity to care for a diverse adult population, manage both acute and chronic conditions, and truly make a difference in your patients’ lives. Requirements
- Connecticut-licensed APRN (FNP or ANP)
- Primary care experience preferred, but strong new graduates are welcome
- Excellent communication skills
- A provider who enjoys teamwork and continuity of care
Responsibilities
- Provide comprehensive primary care to adult patients
- Perform physical exams, diagnose and treat acute and chronic conditions
- Order and interpret diagnostic tests
- Develop and manage treatment plans
- Educate patients on preventive care and wellness
- Maintain accurate and timely documentation in the EMR
