About the position
Theoria Medical is seeking dedicated nurse practitioners and physicians to join our innovative healthcare team. This full-time position offers a unique opportunity to provide high-quality, patient-centered care in skilled nursing facilities (SNFs). The role involves managing clinical care, prescribing medications, and ensuring timely documentation of patient encounters. Providers will work in a supportive community, with access to a network of physicians and providers, and will utilize advanced technology for documentation and patient care. The position emphasizes meaningful connections with patients and staff, fostering a culture of appreciation and professional growth.
Responsibilities
- Rounding and managing clinical care of the population at assigned SNF.
- Prescribing medications and treatments for residents at SNFs.
- Providing high quality, patient-centered, comprehensive primary care services to patients in SNFs.
- Timely documentation of all patient care encounters.
- Available for phone/secure messaging consultations with facility nursing staff.
- Completing documentation in Theoria’s proprietary electronic health record (EHR) system, ChartEasy™.
- Ensuring on-site attendance for scheduled shifts.
Requirements
- Medical licensure to practice in the relevant state.
- Experience in primary care or related field preferred.
- Ability to manage time effectively and attend to patient needs promptly.
- Reliable transportation to travel to various SNF locations.
Nice-to-haves
- Experience with EHR systems such as PointClickCare, MatrixCare, or American HealthTech.
- Previous experience in skilled nursing facilities.
