About The Position

RiverMead is seeking a compassionate and experienced Nurse to serve as a Nursing Coordinator for our Memory Support neighborhood in Peterborough. This leadership role combines hands-on clinical care with team coordination to ensure residents living with memory loss receive exceptional, person-centered support.
The Nursing Coordinator plays a key leadership role by guiding and supporting Nurses, MNAs, LNAs, and PCAs across all shifts. This position ensures continuity of care, promotes strong communication among interdisciplinary teams, and helps maintain a safe, resident-centered environment.

Requirements

• Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of New Hampshire.
• 2–3 years of clinical nursing experience preferred, ideally in memory care, assisted living, or long-term care.
• Experience working with individuals living with dementia or cognitive impairment strongly preferred.
• Demonstrated leadership ability or experience coordinating care teams.
• Strong clinical assessment, care planning, and problem-solving skills.
• Excellent communication and collaboration abilities.

Responsibilities

• Conduct resident assessments and provide specialized nursing care within the Memory Support neighborhood.
• Develop, implement, and update individualized care plans in collaboration with residents, families, and the interdisciplinary team.
• Administer medications and treatments, including wound care, immunizations, and infection management.
• Monitor resident conditions and communicate changes with physicians and healthcare providers to ensure timely care.
• Lead and support nursing and direct care staff while overseeing daily clinical operations and ensuring regulatory compliance.
