About The Position

We are looking for a personable, organized, energetic individual with a clinical background. The Nursing Facility Provider Liaison acts as the primary link between healthcare facilities, physicians, and referral sources to foster strong relationships, promote services, and increase patient referrals. This role requires a deep understanding of healthcare services and the ability to build partnerships that align with organizational goals and enhance patient care specific to the nursing home environment. Key Responsibilities: Relationship Management Establish and maintain positive relationships with nursing facility administrators, clinical teams, and support staff. Act as the primary contact for addressing concerns, feedback, and inquiries from nursing facilities. Facilitate effective communication and collaboration between the organization and providers. Operational Support & Strategic Outreach Support nursing facilities with administrative tasks and troubleshoot issues related to care delivery, documentation, and reporting to foster harmonious relationships with our practice Identify and target new facilities to increase referrals and grow service lines. Conduct regular visits to nursing facilities to promote organizational services and maintain engagement. Develop and execute strategic plans to expand referral networks Education, Training and Promotion Provide education and resources to nursing facilities on our organizational protocols, policy updates, and compliance requirements. Coordinate training sessions and workshops for facility staff to enhance skills and knowledge related to our practice. Educate nursing facility staff about our organization’s services, capabilities, and new programs. Organize presentations, informational sessions, and events to enhance awareness and understanding of services. Provide marketing materials, brochures, and other resources for nursing facilities. Data Management and Reporting Prepare reports and presentations for internal leadership to highlight trends, achievements, referral activity, outreach outcomes and areas for improvement. Monitor referral patterns and analyze data to identify trends and opportunities for growth. Provide biweekly reporting on liaison activity such as locations, personnel, visits, etc. Collaboration, Communication, Compliance & Standards Work closely with marketing, clinical, and administrative teams to align outreach efforts with organizational goals. Communicate feedback from physicians and facilities to internal teams to improve services and processes. Participate in team meetings and contribute to the development of new programs and initiatives. Qualifications: Education: Bachelor’s degree in healthcare administration or nursing Experience: Minimum of 1-3 years of experience in healthcare operations, provider relations, or a related field. Familiarity with nursing facility operations, regulations, and standards of care. Administration or Director of Nursing experience in nursing home or long-term care setting Skills: Strong interpersonal and communication skills. Ability to analyze and solve complex problems. Ability to build and maintain professional relationships. Proficiency in Microsoft Office and CRM software. Analytical skills for interpreting data and identifying opportunities. Requirements

  • Bachelor’s degree in healthcare administration or nursing
  • Minimum of 1-3 years of experience in healthcare operations, provider relations, or a related field.
